From Significance of the study to Public health relevance:
Given that: (1) more than one seventh of the global population suffers from hearing impairment; (2) majority of hearing impairment occurs in low and middle income countries, for instance, 3 in 1000 from developed countries, while 6 in 1000 from developing countries suffer from hearing impairment; (3) damage to the auditory hair cells (HCs) is the principal reason for hearing loss; (4) only in 50% cases hearing impairment can be cured; and (5) the global annual economic cost spent for unaddressed hearing loss is estimated to be in the range of $750–790 billions, there is an urgent need to find: (i) a way to promote regeneration, growth and proliferation of auditory hair cells in mammalian cochlea; (ii) a cheaper alternative to the existing expensive drugs; (iii) a side-effect-free natural product-based drug; and (iv) a way to cure, not just treat, hearing impairment.
From research findings to Therapeutic opportunity:
This study provides, for the first time, mechanistic insights into how a pharmaceutical mixture encompassing Lactolose (a non-absorbable sugar used in the treatment of constipation an hepatic encephalopathy) Cholecalciferol, Ajoene and DIM (LCAD) may aid in the treatment of hearing impairment.
LCAD, by increasing the expression of its target genes, it may increase the expression of ATG5 (Autophagy-related protein 5). Thereby, it may: (1) prevent degeneration of adult hair cells; (2) prevent aggregation of polyubiquitinated proteins and p62/SQSTM1 in inclusion bodies; (3) promote maintenance of hair cells morphology and hearing acuity; and (4) prevent hearing loss. Thus, pharmacological mixture encompassing ” Lactolose, Cholecalciferol, Ajoene and DIM (LCAD) or their analogues, either alone or in combination with other drugs, may be used to treat deafness (Fig.1).
