From Significance of the study to Public health relevance:

Given that: (1) more than one seventh of the global population suffers from hearing impairment;  (2) majority of hearing impairment occurs in  low and middle income countries, for instance, 3 in 1000 from developed countries, while 6 in 1000 from developing countries suffer from hearing impairment; and in india, 6.3% of population suffer from auditory loss; (3) damage to the auditory hair cells (HCs) is the principal reason for hearing loss;  (4) only in 50% cases hearing impairment can be cured; and (5) the global annual economic cost spent for unaddressed hearing loss is estimated to be in the range of $750–790 billions, there is an urgent need to find: (i) a way to promote regeneration, growth and proliferation of auditory hair cells  in mammalian cochlea; (ii) a cheaper alternative to the existing expensive drugs; (iii) a side-effect-free natural product-based drug; and (iv) a way to cure, not just treat, hearing impairment.


From research findings to Therapeutic opportunity: 

It is known for decades that putting a few drops of mustard oil in your ears may aid in preventing hearing loss. However, is there a scientific basis to it?

The use of mustard oil, in treating hearing impairment, is known, in ayurvedic medicine, in India, for centuries. However, the mechanistic basis of which remains completely obscure, and remains unaddressed, at the molecular level, which the specialists in academic medicine wish to know, in order to accept this as a reliable mode of treatment, for decades.

This study provides, for the first time, mechanistic insights into how commonly used mustard oil and its constituent may aid in the treatment of hearing impairment.

Mustard oil-derived compounds, such as   Allyl isothiocyanate (AITC), by increasing the expression of its target genes, it may increase the expression of ATG5 (Autophagy-related protein 5). Thereby, it may: (1) prevent degeneration of adult hair cells; (2) prevent aggregation of polyubiquitinated proteins and p62/SQSTM1 in inclusion bodies; (3) promote maintenance of hair cells morphology and hearing acuity; and (4) prevent hearing loss and presbycusis. Thus, pharmacological mixture encompassing Allyl isothiocyanate (AITC), Cholecalciferol, Ajoene and DIM (AITCCAD) or their analogues, either alone or in combination with other drugs, may be used to treat deafness/presbycusis (age-related hearing loss) (Fig.1).

Given the mechanistic basis as to how mustard oil or its component AITC (fig.1) aids in attenuating or decreasing or alleviating presbycusis and hearing loss, physicians/ENT specialists may consider encouraging their patients to undergo mustard oil-based therapy.

Together, this study, for the first time,  paves, not only for the way to identify new drugs that attenuate hearing-loss, from mustard oil or seeds, but also suggests, as to how mustard oil-based therapy may function as an adjuvant therapy, if not replace, for patients suffering from  presbycusis and hearing loss (Figure 2-4),
