Introduction: What they say:

A study from the Department of Cardiology, Boston Children’s Hospital, Harvard Medical School, Boston, Massachusetts, USA; and Harvard Stem Cell Institute, Harvard University, Cambridge, Massachusetts, USA shows that “Trbp regulates heart function through microRNA-mediated Sox6 repression.“This study was published in the 2 June  2015 issue of the journal “Nature Genetics” (the number 1 research journal in Genetics with an impact factor of 29.648) by Prof. Da-Zhi Wang & Jian Ding, and others.

On the foundation of this interesting finding, Dr L Boominathan PhD, Director-cum-chief Scientist of GBMD, reports that: Natural product-based therapy for Cardiomyopathy: Apple extract induces the expression of Trbp/miR-208, restores the balance between slow- and fast-twitch myofiber proteins and alleviates Cardiomyopathy via up regulation of its target gene

Significance:

Given that: (1)  cardiovascular disease is the leading cause of death worldwide; (2) in India, in 2004, 14.6 lakhs deaths (14% of total deaths) were due to ischemic heart disease;  (3) the global economic cost spent in the treatment of cardiovascular disease in 2011 was little more than 10 billion US dollars, there is an urgent need to find: (i)  a way to induce regeneration of adult cardiomyocytes that were lost in Myocardial patients; (ii) a cheaper alternative to the existing expensive drugs; and (iii) a side-effect-free natural product-based drug.

What is known?

A number of studies suggests that Cardiomyopathy is the result of altered gene expression of contractile proteins. Trbp, an RNA binding protein, has been shown to be required for normal functioning of heart contractions. Prof Da-Zhi Wang‘s research team has recently shown that inactivation of Trbp in mice leads to progressive cardiomyopathy and heart failure.

From Research findings to Therapeutic opportunity:

This study suggests, for the first time, a Natural product-based therapy for Cardiomyopathy.

I have published earlier that Apple extract: (1) improves myocardial function after myocardial infarction via up regulation of PNUTS/PPP1R10; (2) increases SOX2 expression and promotes innate immunity against bacterial infection; (3) promotes insulin-sensitized state via up-regulation of reprogramming protein Lin28; and (4) increases life span via up-regulation of its target gene BubR1.

This study suggests, for the first time, that Apple extract, by increasing the expression of its target gene, it may: (1) increase the expression of Trbp and miRNA-208; (2) decrease the expression of Sox6; (3) increase the expression of cardiac slow-twitch myofiber proteins; (4) decrease the expression of skeletal fast-twitch myofiber proteins;  (5) restore the balance between slow- and fast-twitch myofiber proteins; and (6) alleviate cardiomyopathy.

Apple tree. Apple extract promotes innate immunity against bacterial infection via up regulation of Sox2

Apple tree. Apple extract increases  Trbp and miRNA-208 expression,  decreases  Sox6 expression, and alleviates cardiomyopathy.

Thus, pharmacological formulations encompassing “Apple extract or an active compound isolated from it” may be used to inhibit progressive cardiomyopathy and lethal heart failure. Given that Apple extract has a number of health benefits, as discussed above, it may be an agent of choice to treat cardiomyopathy and heart failure.
